Outreach
Dream Care offers outreach to children and youth aged 4–24 — including those experiencing homelessness or without access to trauma-informed, harm-reduction support.
Some of the young people we work with through outreach may also have the opportunity to live in our home. Others we support alongside their existing service team. Either way, healing starts once relationship is built.
What outreach offers
- Trauma-informed relationship building
- Harm-reduction education and safety planning
- Cultural connection — Elders, ceremonies, land-based teachings
- Support navigating Children's Services, AHS and EPS
- Life-skills coaching (budgeting, housing, employment)
- Referrals to counselling, addictions and mental-health supports
- Advocacy alongside young people in meetings and case planning
- Crisis response and safety check-ins
- Transportation to appointments and cultural events
- Connection to education and training pathways
- Coordination with existing service teams
- Ongoing trusted-adult presence
